Please take a few minutes to print and complete these forms.  Bring them with you to your first visit, or come a few minutes early to complete them at the office.  If you have mental health coverage for a plan that we are on, feel free to call, fax, or email us your insurance information.  This will allow us to review your benefits, or gain any necessary precertification from your insurance company prior to your first appointment.

Our office building is two blocks south of Highway 59 at Kirby And Bissonnet.  Click on the "Directions" link below to see a map of our location.  You will see a bubble containing our address 5311 Kirby, Houston, TX 77005.  Simply click the link inside the bubble that reads "To here" and input your starting address.  Driving directions will then appear in the column to the left of the map.  Please note that our office is in Suite 204.
